The aim of this trial was to determine standard of care and gain evidence of intra- and postoperative outcomes for patients undergoing relaparotomy compared to primary laparotomy.
In this single-center controlled clinical trial, adult patients scheduled for elective abdominal surgery via relaparotomy or primary laparotomy were consecutively screened for eligibility. The perioperative course was monitored prospectively in five study visits during hospital stay and one study visit 1year after surgery. Intraoperative standards, short and long-term outcomes were statistically explored at a level of significance of 5%.
A total of 131 patients with relaparotomy and 50 patients with primary laparotomy were analyzed. In the relaparotomy group, the access to the abdomen took longer (23.5min vs. 8.8min; p?=?&lt;?0.001) and the peritoneal adhesion index was higher (10.8 vs. 0.4; p?=?&lt;?0.001). Inadvertent enterotomies were more frequent in the relaparotomy group (relaparotomy 0.3 versus primary laparotomy 0.0; p?=?0.002). The overall comprehensive complication index and rates of surgical site infection and wound dehiscence with evisceration were not different between the two groups. In this first prospective comparison of relaparotomy with primary laparotomy, inadvertent enterotomies were more frequent in the relaparotomy group. However, contrary to previous retrospective studies, the risk of complications and incisional hernias was not increased compared to primary laparotomy.

We investigated the outcome of patients who underwent reexcision for ESTSTS compared with primarily resected at our institution and the prognostic impact of microscopic residual disease (MR) in the reexcision specimen.
Primary ESTSTS patients surgically treated at our institution between 1997 and 2017 were divided in three groups primarily resected (A), reexcised after macroscopically complete UE (B), and incomplete UE (C). Weighted overall survival (OS), crude cumulative incidence of local relapse (CCI-LR), and distant metastasis (CCI-DM) were calculated and compared. In group B, multivariable models were performed to assess factors associated with the outcomes.
A total of 1962 patients were identified 1076,697 and 189 in groups A, B, and C, respectively. Overall median follow-up was 85 months. Seven-year weighted-OS was 73.8%, 84.1%, and 80.7% (p &lt; 0.001) for groups A, B, and C respectively. Seven-year CCI-LR and DM were 5.0% and 25.3%, 12.1% and 15.8%, and 13.6% and 29.4% (both p &lt; 0.001) for groups A, B, and C, respectively. At multivariable analysis, the presence MR was associated with LR (p &lt; 0.001) but not with OS nor CCI-DM.

However, it is undetermined whether emojis recruit the same perceptual and cognitive processes for identification and integration during reading as do words. To investigate whether emojis are processed like words, we used eye tracking to examine the time course of semantic processing of emojis during reading. Materials consisted of sentences containing a target word (e.g., coffee in the sentence "My tall coffee is just the right temperature") when there was no emoji present and when there was a semantically congruent (i.e., synonymous) emoji (e.g., the cup of coffee emoji, ) or an incongruent emoji (e.g., the beer mug emoji, ) present at the end of the sentence. Similar to congruency effects with words, congruent emojis were fixated for shorter periods and were less likely to be refixated than were incongruent emojis. In addition, congruent emojis were more frequently skipped than incongruent emojis, which suggests that semantic aspects of emoji processing begin in the parafovea. Finally, the presence of an emoji, relative to its absence increased target-word skipping rates and reduced total time on target words.
